Correction of a Credit Report (Follow-Up) - Free

This is a form that permits a person to draft a letter to a local credit bureau or national credit agency to remind the credit organization of their obligation to investigate disputed information and respond to the author within thirty day of a letter giving notice of inaccuracies in the author's credit report.

This form allows the author to enter his name, address, social security number, and the date of the last letter. The form also pre-addresses the letter after the author chooses from the three major Credit Reporting Agencies in the United States. If the author must contact a local agency, the author may also enter the agency's address.
