This form should be used when you need to obtain a copy of a Notice of Commencement filed in Georgia. Upon receipt, the general contractor has 10 days in which to supply you with a copy of the Notice of Commencement (if one was filed). After you submit the questionaire, the completed form will be sent to you within 1 business day for you to print out on your letterhead and for you to mail via certified mail.
