Request for Advanced Sick Leave - Free

This form should be used by employees who know in advance that they will need to take sick leave. Such an advanced request should be accompanied by a doctor's certification of the need for the leave. This form should not be used by employee's seeking leave under the Family Medical Leave Act (FMLA).
How it works:

1. You will answer a questionnaire giving us the information we need to complete your document.
2. We will upload your completed document to your secure client portal.
3. You will be able download your document, print it, out and store the final copy online indefinitely.
4. Additional instruction for use of the completed document is included in the price.
