Authorization for Minor's Medical Treatment - Free

This document can be used to generate an Authorization for a Minor's Medical Treatment. This authorization form will allow you to appoint a temporary guardian and dictate the course and scope of medical treatment your minor will be provided. This agreement also includes a Medical Treatment Contact List.
How it works:

1. You will answer a questionnaire giving us the information we need to complete your document.
2. We will upload your completed document to your secure client portal.
3. You will be able download your document, print it, out and store the final copy online indefinitely.
